Does the Use of the Dermabond Prineo Skin Closure System Decrease Wound Breakdown in Breast Reduction Patients?

Goals/Purpose:
Dermabond Prineo Skin Closure System is a relatively new
product manufactured by Ethicon which utilizes a self-adhering mesh that is
applied to the approximated skin edges to provide temporary skin edge
alignment and a liquid topical skin adhesive is applied over the mesh to
achieve skin closure. It is intended to hold closed easily approximated
skin edges of wounds from surgical incisions. It is intended to be used in
conjuction with deep dermal sutures and has been shown to have the benefit of
reducing final closure time by as much as 75%. This product was used at our
facility by one surgeon for all of the patients undergoing breast reductions.

Methods/Technique:

This study is a retrospective chart review. A single surgeons
breast reduction cases were pulled for a consecutive 2 year period.
Exclusion criteria was prior history of breast cancer, prior breast
rediction, or any radiation to the chest previously. Demographic data
,pre-operative data, operative and post-operative data, as well as
complications were collected.

Results/Complications:

There was a total of 30 patients who had been closed with the
Dermabond Prineo Skin Closure System and 24 patients closed without the
system. The age, race, BMI and schnur scores, as well as resection scores
were similar across these two groups. It was noted that wound dehiscence,
with respect to the T-Zone region occurred in 66.66% of patient’s who had
closure with sutures alone versus 20% % in patients who had been closed
with the Dermabond Prineo device. It was also noted that patients who were
smokers, had Diabetes, were on anticoagulants were more likely to have
wound healing problems despite the use of the closure device.

Conclusion:

The Dermabond Prineo Skin Closure System significantly reduced wound dehiscence. It is a safe and efficacious product to use.
