Scar Camouflage with Micropigmentation Tattoo

Goals/Purpose: Surgical scars are not always predictable. Facial scar management has improved with topical silicone products and microneedling. However, scars refractory to these management alternatives can represent a challenging problem. While surgeons can camouflage scars with their placement, some scar locations are more visible. Advances in micropigmentation have allowed scar management with complexion-matching tattoo ink.

Methods/Technique: Clinically visible scars were evaluated by the operative surgeon and determined to be refractory to established management. Alternative management was offered for either continued conservative care, surgical re-excision, or micropigmentation tattoo. The scar was assessed by the tattoo artist to determine if the scar was appropriate and met the requirements of micropigmentation tattooing. The scar was assessed for tone and pigment of the surrounding skin to determine which tattoo ink color(s) were to be used. Color-matching was performed with multiple inks to optimally camouflage the scar. Anticipation of subsequent fading and pigment changing were taken into consideration for pigment selection. Post-procedure care includes cleansing the tattoo with a gentle soap and water 3 times daily for the first 3-5 days, followed by application of a thin layer of A&D ointment.

Results/Complications: Five patient were treated and indicated a high degree satisfaction with scar camouflage tattooing after 28 days and required no additional treatment.

Conclusion: Surgical scars are an unavoidable element of facelifts. Some patients experience noticeable scars, despite conservative measures of silicone scar cream and microneedling. By camouflaging surgical scars with tattoos we can offer alternatives for treatment beyond basic conservative measures. Skilled tattoo artists complement aesthetic rejuvenation following surgery with advanced techniques for camouflaging unsightly scars.
