Use of Aveli As an Effective Tool for Cellulite Reduction
Methods/Technique: Cellulite is an aesthetically undesirable cosmetic problem for most post-adolescent women. It is largely observed in the gluteal and thigh regions with its 'orange-peel' appearance. Differences in the fibrous septae architecture that compartmentalize the adipose tissue have recently been reported in women with cellulite compared with men. Various treatment options are available for cellulite reduction. In this study, we evaluated the efficacy of Aveli – a novel device for cellulite reduction. Aveli consists of a handle and a distal end. A pre-operative Cocktail containing anxiolytic, pain medication and oral antibiotic was given to the patient 30 minutes prior to the surgery. The cellulites were marked pre-operatively while patients are standing. Tumescent solution containing 400 mg of lidocaine with epinephrine per 250 ml of bag was used for local infiltration. About 150 tumescent was injected in each buttock from a stab incision in intragluteal crease. The distal end of the device is then advanced into subcutaneous tissue. An integrated light source provides illumination and allows the user to track and advance to the proposed cellulite. The distal end contains a blade and a blocker forming a hook. When the Handle is moved in a retrograde fashion, the Hook next to fibrous septa captures the septa resulting in tugging. The user feels the resistance, confirming that septa under a cellulite depression have been identified and then exposes the Blade at the Distal End. The user pushes the skin distally with the free hand while maintaining the device stable or applies additional retrograde motion with the device to release the fibrous septa. The user then retracts the Blade and the Blocker into the device, allowing removal without further tissue engagement. The user can verify all contributing septa have been released by passing through the area again with the Hook. The step is repeated for each visible cellulite depression. All our patients had only 1 treatment which was considered definitive.
Results/Complications: 10 patients were operated for cellulite reduction using this approach from July 2022- November 2022. All patients were female. The mean age was 35.4 years (24-56). Mean follow up for our study was 57 days. The patients, overall, tolerated the procedure very well and we did not have to abort any procedure. 1 patient complained of bruising that persisted beyond 4 weeks. It resolved by 8 weeks. In all other patients, the bruising mostly subsided by 2 weeks. There was no incidence of infection during our follow up period. Overall, the patients were very happy with the outcomes. Attached is a picture of a representative patient who is 6 weeks out from this procedure.
Conclusion: Aveli Precision Cellulite Release Device is an effective tool for reduction of cellulite. The advantages of using this device is that it can easily be done under local anesthesia and it requires only one treatment session for definitive results.
