4880 Conical Plication for Breast Augmentation

Goals/Purpose: Adequate fullness of the superior part of the breast is one of the most important indicators for the aesthetically pleasant, younger looking breast. In this study, we create a new plication technique for the upper part of the breast as conical plication.

Methods/Technique: We performed mastopexy operations with superior conical plication technique with 2/0 PDS in the upper pole of the breast for the creating of superior fullness with or without breast prosthesis. We used modified total posterior pedicle NAC safety and to preserve lactation and nipple sensation and maximum mobility. We performed auto-augmentation in patient who had adequate breast parenchyma or augmentation with breast implants in patients with significant breast atrophy.  Preoperative and postoperative standard measures of the breast including the superior pole fullness were compared.

Results/Complications: Our technique was performed in 16 patients. 56 percent of the patients were operated as auto-augmentation mammaplasty and rest of them operated as mastopexy with breast implants. None of the patients had such major complications as necrosis, partial or total NAC loss. None of them required revision surgery. Superior fullness breast was enhanced 83 % with this approach.  Breast ultrasonography and mammography were performed to evaluate effect of the plication to the breast parenchyma in early and long term of the post-operative period. We never observed any abnormalities in long term in the breast parenchyma.

Conclusion: Our approach is a versatile, safe, reliable technique which creates adequate superior breast fullness with maximum preservation of functions, can be employed with breast prosthesis as mastopexy operation or auto-augmentation mammaplasty without breast prosthesis.
